Care & Rehab – Ostrander Minnesota community make it possible to maintain loving connection with others which offers independence as well as safety and security. Our Licensed Professional Nurses and staff are on the floors 24 hrs a day, 7 days a week to meet specific needs of each person.
Skilled Nursing
We provide continuous skilled nursing care on an extended basis to those requiring more help than a home caregiver can manage. This care includes a physician, dietary services, an activity program and pharmaceuticals. Our professional staff is always on hand to meet our resident’s individual needs in a personal and respectful manner.
Rehabilitation Programs
Care & Rehab – Ostrander Rehabilitation Program supports your transition from hospital to home. Once discharged from Care & Rehab’s Rehabilitation Program, we also offer rehabilitation transportation and an Outpatient Therapy program.
Care & Rehab’s therapy team’s main focus is to help restore your physical and/or mental function in as short a time period as possible. Our extensive experience, alongside a care plan tailored to promote function and independence, helps guide you to your goal.
